Notice of Structural Defect
Claim Form - This form is for homeowners who are experiencing a structural
defect and are currently covered under StrucSure's 10-year structural home warranty.
Please note that you may be required to pay a warranty service fee as part
of your claim process. If you have questions regarding this fee or your claim,
please contact Zoe Ann Chavous, StrucSure’s Claims Administrator, at 1.877.806.8777

Request for Warranty
Performance Form (Workmanship/Materials and/or Delivery Portions of Systems) - This
form is for homeowners who are currently experiencing a warranty issue with
Workmanship / Materials and/or the Delivery Portion of Systems and previous written
attempts to resolve those issues with their builder have failed. Once completed,
this form must be sent to your builder and a copy must be sent to StrucSure Home
Warranty's Warranty Service Division. Please note that you may be required
to pay a warranty service fee as part of your claim process. If you have
