Notice of Structural Defect Claim form - This form is for homeowners who are experiencing a structural defect and are currently covered under StrucSure's 10-year structural home warranty. Please note that you may be required to pay a warranty service fee as part of your claim process. Notice of Complaint form - This form is for homeowners who are currently experiencing a warranty issue and previous written attempts to resolve those issues with their builder have failed. Once completed, this form must be sent to your builder and a copy must be sent to StrucSure Home Warranty's Warranty Service Division.
